2. Types of Information Collected
We retain two types of information: Personal Data This is data that identifies you or can be used to identify, contact you or process applications on your behalf and may include your name, address, email address, user IP addresses in circumstances where they have not been deleted, clipped or anonymised, telephone number and billing information. Such information is only collected from you if you voluntarily submit it to us or have authorised us to obtain it. 7. Updating, Verifying and Deleting Personal Data
You may inform us of any changes in your Personal Data, and in accordance with our obligations under the Data Protection Acts 1988 and 2003, Directive 95/46/EC of the European Parliament and of the Council of 24 October 1995 on the Protection of Individuals with regard to the Processing of Personal Data and on the Free Movement of Such Data, and any implementing and/or amending legislation as may be adopted in Ireland from time to time, we will update or delete your Personal Data accordingly. 9. Freedom of Information
As most of the information requested from the Clinic comes under the category of the client's own personal information they have very strong rights of access under the Freedom of Information Act. The majority of requests come from adults who attended the Clinic as children. Once a client is over the age of sixteen they can request a copy of their records by writing to the Medical Records Department, stating their date of birth, the address they were living at when they attended the clinic, their current address and a photocopy of some form of photo identification such as a passport or driver’s licence. Once the request is received in writing, a client can expect to receive a copy of their records within four weeks.
The right of access to medical records under the above Acts is restricted in some very limited circumstances, where the health and mental well being of the individual might be affected by obtaining access to the data. In such circumstances, it is the policy of the Clinic to communicate this information through a nominated health professional, such as a General Practitioner, Psychiatrist, Clinical Psychologist or Social Worker.
10. Data Protection
Under the Data Protection Act, the Clinic has a responsibility to hold personal information relating to clients in a responsible manner. We are guided by the Regulations of the Act in ensuring that the information we hold on computer or in a manual filing system is accurate, is up-to-date, may not be given to someone not entitled to see it, is kept safe and secure and personal privacy is maintained at all times.
